I could see something like this working well with a math story problem as well, perhaps with a few lines for writing what is known, and space at the bottom for visualization (bar model?) or solving.

CAPE Stuff
I went to the CAPE Reading meeting last week, and will be sharing a few things on-line as I have time. One of the pieces I thought might be of value was this Reading Level Correlation Chart. This is a great chart as you look for appropriate reading material for your students.
